Please complete the request form to confirm your booking. This experience is only open to overnight guests of Mandalay Bay or THEhotel, please have your room confirmation number ready before completing this form.

Diver Information

All divers must present a valid certification card from a recognized dive instruction agency. (PADI, NAUI, etc.)
If Date of Last Dive has been more than 12 months ago, a SCUBA refresher course will be required prior to arrival
All divers must sign and answer health related questions specific to diving
All divers will be required to sign a liability waiver
All divers will be attended by aquarium staff at all times
Divers will be provided with all dive equipment during their experience, wet suit, BCD, fins, chain maille, etc. No outside equipment will be allowed with the exception of a swimsuit and face mask.

Cancellations:

A Cancellation or date change must be requested more than 72 hours in advance. There will be a cancellation or change fee of $75 per diver. Late cancellations and no-shows will not be refunded or rescheduled.
